What you can expect in this role:
- Provide comprehensive care across multiple settings—telehealth, in-office visits, and on-site care at assisted living facilities
- Ability to diagnose routine conditions and consult (or refer to) physicians for more complex diagnoses.
- Treat patients by following best practice protocols and consult with physician leaders if you deem it necessary to modify the care pathways.
- Communicate with your patients as necessary to ensure we offer a delightful experience.
- Manage patient treatment plan and provide support and education to patients on the way.
- Educate patients about Ascend's broader offering, conduct health risk assessments and guide them on their journey to longevity and happiness.
- Leverage cutting-edge technology to streamline workflows, reduce administrative burdens, and focus on delivering high-quality care

Requirements
- Master's degree from an AMA accredited school for Advanced Practice Providers.
- Current, active PA or NP licensure in State of employment is required.
- Internal Medicine or Family Medicine Board Certification (or board eligible)
- Active, non-probationary, unrestricted Georgia State License
- Active DEA license or eligibly to obtain
- 1+ year clinical experience in internal medicine or primary care practice setting. Experience with geriatric care a plus.
- Technological proficiency—comfortable using EMRs and digital health tools to enhance care delivery
