Advanced Practice Provider (Assisted Living)- Nurse Practitioner/Physician Assistants

We are seeking a dedicated provider with a passion for building meaningful relationships and providing compassionate care to assisted living patients. This role offers flexible hours, high compensation potential, and an opportunity to make a lasting impact through empathetic patient care.

Key Responsibilities:

Build and maintain strong relationships with patients, their families, and facility staff, fostering trust and collaboration.
Provide compassionate care to assisted living patients, addressing their physical, emotional, and social needs.
Communicate effectively and empathetically with patients and their families to ensure understanding of treatment plans and care goals.
Collaborate closely with facility staff to ensure seamless care and support for patients.
Maintain a flexible schedule, balancing patient visits with some in-office hours (ideally every other Friday from 9 AM - 1 PM).
Apply hospice or home health experience to deliver exceptional care to patients requiring complex medical attention.

Qualifications:

Licensed Nurse Practitioner or Physician Assistant with 3+ years' clinical experience, preferably in home health or hospice care.
Master's degree from an AMA accredited school.
Active, non-probationary, unrestricted Georgia State PA or NP License
Active unencumbered DEA License for Georgia OR eligibility to obtain
Ability to work in North Metro Atlanta area- Woodstock/Canton area.
